NHHS SHARK TANK

Welcome to the NHHS SHARK TANK!

We are pleased to announce an opportunity for up to six (6) community groups/individuals to apply for up to $300 per group/individual, to propose a project on "Community Health and Well-Being."

Your proposal should speak to a creative and innovative way that you plan to promote health in your community.

Eligibility Requirements

• In order to be eligible to enter into the Shark Tank; you must commit to identifying and connecting five (5) pregnant women, five (5) parenting under 2 moms and ten (10) fathers/fathers-to-be, to the New Haven Healthy Start Program (NHHS). Download referral form here.
• You will have from February 21, until, March 9, 2018 to make these connections. Once the individuals that you have identified have been confirmed, and connected to NHHS; you will have until March 30, 2018 at 5:00 pm to submit your proposal.
• During our April 18th NHHS Consortium/CAN meeting; you will enter the "Shark Tank" and our meeting participants will determine which project gets funded. Please, be creative with your project and projects with partners are STRONGLY encouraged.
