New Haven Healthy Start Provides Care Coordination

New Haven Healthy Start Consortium Members

New Haven Healthy Start (NHHS) continues to provide a care-coordination model and referrals to medical facilities. Since 2002 , it has served over 13,000 pregnant women and 7,000 infants. NHHS is supported by a grant of $4.5 million awarded in 2009 by the Department of Health and Human Services, the Health Resources and Services Administration, the Maternal and Child Health Bureau and the Division of Perinatal Health Systems. This grant will continue through 2014. NHHS was an outgrowth of the work of the Commission on Child and Infant Health, convened by The Community Foundation in 1985 to address the high infant mortality and morbidity rates in New Haven. Because African-American women living in New Haven are 2.5 times more likely to experience an infant death and more likely to have preterm and low-birth-weight babies, NHHS focuses on infant mortality and the elimination of racial and ethnic disparities in birth outcomes.


New Haven Healthy Start is a program that helps pregnant women and mothers deliver healthy and happy babies. Its primary goal is to reduce the infant mortality and morbidity rate in the City of New Haven. The program was awarded a fourth round of federal funding in 2009 to:

  • Core Messageshelp reduce New Haven’s remaining health disparity in infant mortality;
  • increase its emphasis on multi-pronged outreach strategy;
  • strengthen connections with new community resources; 
  • enhance case management capabilities for working with women at high-risk of poor birth outcomes. (Special outreach will be made to teens, the homeless, and those who lack insurance coverage and who have had previous poor birth outcomes).
